Creating a progress chart enables you to track a client's progress over time in a manner that is visual and easy to understand. A progress chart can be a line graph, a bar graph, a spreadsheet, or any other format that helps you to organize your data. Below are some simple templates that you can print out and use to track progress. They are very easy to use and tracking data will only require seconds each day. These are great to submit with your monthly or quarterly summary reports.

Example #1

This example includes a blank template and a completed template. The chart measures the accuracy of how well the task was completed. Simply match up the date with the percent of accuracy. Then put an X in the box. If no data was collected on a particular date, leave that column blank. At the end of the month, connect your lines. As you can see, in this example, Jane made considerable progress going from 10 percent accuracy to 70 percent accuracy. However, on the 23rd of the month, she did quite poorly. You would want to explain what happened that date when you write your monthly or quarterly summary.

This example includes a blank template for you to measure number of minutes a task was completed. This could also be modified to display number of times a task was successfully completed.

This example includes a partially completed template for you to measure what level of support was provided for each step of a particular task. It provides more detailed information than the previous examples. At the end of the day, the daily score can be calculate for the total task. At the end of the month, the total score for each step can be calculated, showing you at a glance which steps the person is most struggling with.
